The parent/guardian accepts that there are risks associated with both playing and
training for football and that neither Hook United Youth FC, its office bearers or volunteers will be held liable for any injuries incurred. They also understand that Hook United Youth FC does not carry insurance for its members.
The parent/guardian will ensure that their child wears shin pads for both training
sessions and matches.
The parent/guardian acknowledge that Hook United Youth FC has a 'NO jewellery
whilst playing' policy and will ensure that their child removes ALL jewellery before
training and match play.
The parent/guardian confirms that they have received and familiarised themselves
with the contents of the HUYFC member's pack including codes of conduct for
players, managers and parents (available on the website at www.huyfc.com) and
agree to ensure that the member they are signing for and his/her immediate family
will be bound by and will observe at all times, the HUYFC codes of conducts and
guidelines it contains. The parent/guardian accepts that failure to do so will result in
termination of membership.
The parent/guardian confirms that they will undertake to ensure that all visitors
attending matches at their invitation will be made aware of the supporters' code
and asked to observe it.
The parent / guardian also acknowledge that all children under the age of 16 must
be dropped off and collected from training and matches by a responsible adult.
At times the Club may wish to take photos or videos of the team or individuals in it.
We adhere to The FA Guidelines to ensure these are safe and respectful and used solely for the purposes for which they are intended, which is promotion and
celebration of the activities of the Club and for training purposes.
